
Individuals who received a prerecorded voice message from Nationwide Mutual Insurance Co. about the renewal or expiration of their pet insurance policy between Jan. 6, 2021, and Oct. 22, 2025, may be eligible to claim up to $17.50 from a class action settlement.
Nationwide Mutual Insurance Co. agreed to pay $1.4 million to resolve a class action lawsuit alleging it violated federal and Florida law by sending prerecorded marketing calls to consumers without proper consent.
Who can file a claim?
Class members must meet all of the following criteria:
- They received at least one prerecorded voice message from Nationwide Mutual Insurance Co. on their cell phone.
- The message concerned the renewal or expiration of their pet insurance policy.
- The call occurred between Jan. 6, 2021, and Oct. 22, 2025.
- They are located within the United States.
How much can class members receive?
Eligible class members who submit a valid claim can receive up to $17.50. However, the final payment amount may be less, depending on the number of valid claims submitted and the deductions for administrative costs, attorneys’ fees and the service award for the class representative.
How to claim a settlement payment
Class members can file a claim online or download, print and fill out the PDF claim form and mail it to the settlement administrator. The claim deadline is March 11, 2026.

Each class member can only submit one claim despite the number of calls they received.
What proof or documentation is necessary to submit a claim?
To file an online claim, class members must provide the unique ID and PIN located on the settlement notice they received via postcard or email. Those who wish to mail the printable claim form only need to provide their unique ID. Class members who did not receive an email or postcard notice or lost theirs should contact the settlement administrator at 1-877-774-4738.

The settlement administrator will mail paper checks to eligible claimants.
$1.4 million settlement fund breakdown
The $1,400,000 settlement fund includes:
- Settlement administration costs: $100,000
- Attorneys' fees and costs: Up to $490,000
- Service award to class representative: Up to $2,500
- Payments to eligible class members: Remainder of the fund

When is the Blizzard v. Nationwide payout date?
The settlement administrator will mail checks to eligible class members approximately 65 days after the court resolves any appeals and grants final approval to the settlement.
Why did this class action settlement occur?
The class action lawsuit alleged Nationwide Mutual Insurance Co. sent prerecorded voice messages to consumers about pet insurance renewals or expirations without proper consent, allegedly violating the Telephone Consumer Protection Act and the Florida Telephone Solicitation Act.
The company denies any wrongdoing, but both parties agreed to settle to avoid the cost and uncertainty of continued litigation.
