Consumers whose private information was potentially accessed during the SysInformation Healthcare Services LLC data incident that occurred June 3-18, 2023, may qualify to claim up to $5,000 from a class action settlement.

SysInformation Healthcare Services LLC, doing business as EqualizeRCM and 1st Credentialing, agreed to pay $2.8 million to settle a class action lawsuit alleging the company failed to adequately protect sensitive personal information.

Who can file for a data breach payout?

Class members must meet the following criteria:

  • They reside in the United States
  • Their private information was potentially impacted as a result of the SysInformation data breach that occurred June 3-18, 2023.
  • THey received a notice from SysInformation Healthcare Services LLC or its affiliates around April 17, 2024, informing them their personal information may have been exposed.

How much will the class action settlement payment be?

  • Cash payment A: Class members can submit for reimbursement of up to $5,000 for documented out-of-pocket losses traced to the cybersecurity incident.
  • Cash payment B: Class members that do not select cash payment A can submit to receive a flat cash payment of $100.
  • Credit monitoring: All claimants can submit to receive three years of free three-bureau credit monitoring, dark web monitoring, identity theft insurance up to $1,000,000 and identity recovery services.

How to claim a data breach class action rebate

To receive a settlement payment, class members must submit a claim form. Class members can file a claim online or download and print the PDF claim form to fill out and mail to the settlement administrator. All claims must be submitted online or postmarked by Aug. 24, 2025.

What proof or information is required to submit a claim?

  • Class members must provide the unique ID and PIN from their settlement notice.
  • Cash payment A claims require supporting documentation, such as bank or credit card statements, invoices for paid services, police reports or other proof showing unreimbursed expenses or fraudulent charges due to the breach.

$2.8 million data breach settlement fund breakdown

The settlement fund of $2,800,000 includes:

  • Settlement administration costs: To be determined
  • Attorneys' fees: Up to $933,240
  • Service awards to class representatives: Not to exceed $3,000 each for a total of $9,000
  • Credit monitoring costs: Total cost determined by the number of claims filed
  • Payments to approved claimants: Amount remaining after fees, service awards and administration costs
