
Individuals who received a notice stating a November 2024 data incident at LifeBridge Health Inc. may have compromised their private information could be eligible to claim up to $5,000 plus a flat cash payment from a class action settlement.
LifeBridge Health Inc. agreed to pay $575,000 to settle a class action lawsuit alleging negligence, breach of implied contract and breach of the implied covenant of good faith and fair dealing. The lawsuit claimed an unauthorized third party gained access to certain LifeBridge Health computer systems, potentially exposing sensitive patient information.
Who can file a claim?
Individuals are eligible to file a claim if:
- The data incident discovered by LifeBridge Health Inc. on or around Nov. 12, 2024, potentially compromised their private information.
- They received a notice letter from LifeBridge Health Inc. about the 2024 data incident.
The data incident may have compromised the following types of information:
- Names
- Dates of birth
- Dates of service
- Social Security numbers
- Addresses
- Medical record numbers
- Health insurance information
- Limited treatment information
How much can class members receive?
Class members may be eligible for two types of cash payments, depending on the type of claim they submit:
- Cash Payment A – Documented losses:
- Reimbursement for documented losses up to $5,000 per person
- Losses must be related to the data incident and not already reimbursed from another source
- Requires submission of reasonable documentation
- Cash Payment B – Flat cash payment:
- A flat cash payment representing a pro rata share of the remaining net settlement fund after the settlement administrator pays all valid documented loss claims
- Nominally valued at $100, but the actual amount may increase or decrease depending on the number of valid claims submitted
- No documentation required
If a class member submits a valid claim for documented losses, they will also receive the flat cash payment. If they do not submit documentation, they will only receive the flat cash payment.
The settlement administrator will distribute the fund as follows:
- First, it will pay all approved claims for documented losses (up to $5,000 per person).
- It will then divide the remaining funds equally among all class members who submit a valid claim for the flat cash payment.
How to claim a settlement payment
Class members can complete the online claim form or download, print and complete the PDF claim form, then mail it to the settlement administrator. The claim deadline is Feb. 28, 2026.

What information is necessary to submit a claim?
- All claimants must provide the CPT ID from the settlement notice they received. Online claimants must also provide the passcode from the same notice. Those who do not have a CPT ID and/or passcode should email LifeBridgeDataSettlement@cptgroup.com.
- To claim Cash Payment A (documented losses), claimants must provide reasonable documentation of their losses, such as credit card statements, bank statements, invoices, telephone records or receipts. Self-prepared documents like handwritten receipts alone are not sufficient.
- To claim Cash Payment B (flat cash payment), claimants do not need to provide documentation but must submit a valid claim form.

$575,000 settlement fund breakdown
The $575,000 settlement fund includes:
- Settlement administration costs: $50,000
- Attorneys' fees: Up to $191,647.50
- Attorneys' expenses: To be determined
- Service award to class representative: Up to $2,500
- Payments to class members: The remainder of the fund
If there are any funds left over from uncashed checks after 90 days, the settlement administrator will distribute it to the court-approved cy pres recipient, the Center for Urban Families.
As part of the settlement, LifeBridge will also take reasonable steps to further secure its systems and environments at an estimated cost of $200,000 annually. This amount is separate from the settlement fund.

When is the LifeBridge Health Inc. settlement payout date?
The settlement administrator will distribute payments to class members approximately 31 days after the court resolve any appeals and grants final approval of the settlement.
Why is there a class action settlement?
This class action settlement alleged LifeBridge Health Inc. discovered an unauthorized third party accessed its computer systems and potentially compromised patients' private information of patients. The lawsuit claimed LifeBridge Health Inc. was negligent and failed to adequately protect personal information.
The company denies any wrongdoing but agreed to settle to avoid the costs and uncertainty of litigation.
