Mountain West Insurance & Financial Services recently experienced a data breach that has impacted both current and former clients. On March 17, 2025, the insurance company discovered that a threat actor had gained unauthorized access to multiple internal employee email accounts. On Aug. 15, 2025, an investigation revealed that both personally identifiable information (PII) and protected health information (PHI) may have been accessed during the cyberattack.

Exposed information included names, Social Security numbers, dates of birth, driver license numbers, financial account numbers, financial account access information, payment card numbers, payment card access information, passport numbers, electronic signatures, medical information and health insurance information. This combination of data puts impacted individuals at risk for identity theft, financial fraud and medical identity misuse.

While the company has not disclosed the exact number of individuals affected, the wide range of information involved and the nature of client information kept on file suggest the impact could be widespread. Mountain West published a notice of data security incident on its website.

Mountain West Insurance & Financial Services' response

Mountain West Insurance & Financial Services is notifying affected individuals by mail. In addition to require state and federal disclosures, the company is offering free credit monitoring services to individuals whose Social Security numbers were compromised in the data breach.

  • Monitor financial accounts and credit reports for signs of identity theft.
  • Consider placing fraud alerts or credit freezes with the major credit bureaus.
  • Be cautious of unsolicited emails or phone calls requesting personal information.

Mountain West has also set up a response line for individuals with questions at 1-800-939-4170, Monday through Friday 8:00 a.m. to 8:00 p.m. Central Time.
