On Nov. 5, 2025, Denton County MHMR Center reported a significant data breach to the U.S. Department of Health and Human Services (HHS), affecting at least 108,967 individuals in the United States. The breach involved unauthorized access to sensitive information, including both personally identifiable information (PII) and protected health information (PHI).

In December 2024, Denton County MHMR Center became aware of cybercriminal activity on its network. The subsequent investigation revealed that an unauthorized actor gained access to its systems between Dec. 24, 2024, and Dec. 25, 2024.

The types of data exposed included names, addresses, Social Security numbers, bank account information, dates of birth, treating physician names, as well as diagnosis and treatment information. This means that not only were basic identifiers compromised, but also highly sensitive medical and financial data. The exposure of both PII and PHI increases the risk of identity theft, financial fraud, and potential misuse of medical information.

The incident’s scale and the nature of the information involved make it one of the more severe breaches reported in the mental health care sector this year. Details about the specific method of intrusion or the responsible party have not been disclosed publicly, but the broad range of data impacted suggests a significant compromise of internal systems.

The mental health center posted a Notice of Data Breach on its website on Feb. 21, 2025. Impacted individuals have been notified by mail.

Denton MHMR Center's response

Following the discovery of the breach, Denton County MHMR Center took immediate steps to secure its systems and prevent further unauthorized access, including implementing additional security measure to protect against future occurrences.
