
Individuals who received a notice that data incidents at Community Connections Inc. in October 2024 or April 2026 may have impacted their private information could be eligible to claim up to $5,100 from a class action settlement.
Community Connections Inc. agreed to settle a class action lawsuit alleging failed to adequately protect private information, leading to two separate data security incidents.
Who can file a claim?
The settlement considers individuals class members if the data incidents at Community Connections Inc. affected their private information. This includes all people in the United States who received a notice letter from the company about the incidents.
The breaches potentially impacted names, addresses, dates of birth, Social Security numbers, financial information, driver’s licenses or state IDs, medical information and health insurance information.
How much can class members receive?
Class members may be eligible for one or more of the following payments:
- Cash payment A – Documented losses: Up to $5,000 per person for documented, unreimbursed monetary losses related to fraud or identity theft the data incidents caused. Class members must provide supporting documents, such as receipts, invoices, or bank statements.
- Cash payment B – Lost time: $25 per hour for up to four hours (up to $100) for time spent addressing issues related to the data incidents. Class members must provide a brief description of the time spent.
- Cash payment C – Alternative cash payment: $50 for class members who do not claim documented losses or lost time. No documentation is required, but the settlement caps this category at a total of $250,000 for all claimants. If claims exceed this amount, the settlement administrator will reduce payments on a pro rata basis.
All class members will also receive a code for two years of credit monitoring with CyEx Medical Shield Complete regardless of whether they submit a claim for a cash payment.
How to claim a settlement payment
Class members can submit the online claim form or download, print and mail the PDF claim form to the settlement administrator.

What proof or documentation is required to submit a claim?
- Documented losses: Class members must provide third-party documentation, such as receipts, invoices or bank statements showing actual, unreimbursed monetary losses related to the data incidents.
- Lost time: No documentation is required, but class members must briefly describe the time spent.
- Alternative cash payment: No documentation is required.
- Online claims: Class members must provide the unique ID and passcode from the settlement notice they received.

- Electronic payment (class members must file online and select this option)
- Paper check (default for mailed claims or class members who do not select an electronic option)
Settlement fund breakdown
The settlement fund will cover:
- Settlement administration costs: To be determined
- Attorneys' fees and costs: $362,500
- Service awards to class representatives: $2,500 each to the three named plaintiffs ($7,500 total)
- Credit monitoring: Determined by number of valid claims for this option
- Alternative cash payment claims: Up to $250,000
- Documented losses and lost time claims: Determined by number of valid claims for these options
