
Consumers who received notice that their private information may have been compromised in the Jan. 25, 2024, data breach at Family Health Center Inc. may qualify to claim up to $5,000 from a class action settlement.
Family Health Center Inc. agreed to pay $850,000 to resolve two class action lawsuits alleging it failed to adequately protect personal and medical information during a cyberattack. The lawsuits claim the breach exposed sensitive data, such as names, addresses, Social Security numbers and medical information.
Who can file a Family Health Center claim?
Class members must meet the following criteria:
- Their private information was potentially accessible as a result of the Jan. 25, 2024, Family Health Center Inc. data incident.
- They received a notice about the data incident from Family Health Center Inc. or the settlement administrator.
How much can class members get?
- Cash payment A – documented losses: Up to $5,000 for actual, documented out-of-pocket losses due to the data incident. Covered expenses include:
- Bank, credit card and debit card fees
- Overdraft, declined payment and returned check fees
- Costs to replace a driver’s license, Social Security number or other identification
- Fees for credit reports or credit monitoring
- Losses from identity theft
- Cash payment B – flat cash payment: A flat $50 payment
- Credit monitoring services: Two years of credit monitoring, dark web monitoring, identity theft insurance coverage (up to $1,000,000) and fully managed identity recovery services.
Class members may claim either cash payment A or cash payment B but not both. However, they may claim either cash payment in combination with credit monitoring services.
If the total amount of valid claims exceeds the available funds after deducting attorneys’ fees, expenses, service awards and administration costs, payments to class members may be reduced on a pro rata basis.
How to claim a settlement payment
Class members can submit the online claim form or download, print, complete and mail or email a PDF claim form to the settlement administrator. All claims must be submitted online or postmarked by Oct. 8, 2025.

What proof or documentation is required to submit a claim?
- Online claimants must log in with their unique ID and PIN, which is located on the settlement notice they received. Those who are unable to locate their ID & PIN should contact the settlement administrator at info@FHCDataSettlement.com.
- To claim documented losses, class members must submit receipts or other evidence of payment for covered expenses, such as bank statements, invoices or receipts. Self-prepared documents can be used to clarify or support other documentation but cannot be the sole proof.
- To claim a flat cash payment or credit monitoring, class members must check the appropriate box. For credit monitoring, they must also provide a valid email address.

$850,000 settlement fund breakdown
The $850,000 settlement fund covers:
- Settlement administration costs: Up to $75,000
- Attorneys’ fees: Up to $283,305
- Attorneys’ expenses: To be determined
- Service awards to class representatives: Up to $1,500 each
- Credit monitoring costs: To be determined
- Payments to eligible class members: Remainder of the fund

When is the Family Health Center Inc. settlement payout date?
Payments and benefits will be distributed approximately 65-90 days after the court grants final approval of the settlement and any appeals are resolved.
The settlement administrator will send an email to class members with valid claims who elected credit monitoring with information on how to enroll in the program, including the activation code.
Why is there a class action settlement?
The class action lawsuits alleged Family Health Center Inc. failed to adequately protect private information during a cyberattack on Jan. 25, 2024. The plaintiffs claimed the breach allowed unauthorized access to files containing sensitive personal and medical information.
Family Health Center Inc. denies any wrongdoing but agreed to settle to avoid the costs, risks and uncertainties of further litigation.
