Revival Animal Health Inc. customers who received a notice stating their personal and payment information may have been compromised in a cyberattack could be eligible to claim up to $5,000 from a class action settlement.

CommerceV3 Inc. agreed to settle a class action lawsuit alleging it failed to safeguard customer payment information, resulting in a data breach that exposed sensitive personal and financial data.

Who can file a Revival Animal Health claim?

Class members must meet all of the following criteria:

  • They were a customer of Revival Animal Health Inc. in the United States.
  • They received a written notice from Revival stating their name, billing address, payment card number, CVV code and/or payment card expiration date may have been compromised in a cyberattack against CommerceV3 Inc.

The class is estimated to include approximately 82,893 individuals.

Each eligible person is entitled to submit only one claim form for compensation.

How much can class members get?

  • Ordinary losses: Up to $500 for documented, unreimbursed out-of-pocket expenses and up to four hours of lost time spent troubleshooting the incident at $20 per hour (included within the $500 cap). These losses include bank fees, postage, gas for local travel and fees for credit monitoring or identity theft insurance purchased between Nov. 24, 2021, and May 29, 2025.
  • Extraordinary losses: Up to $5,000 for proven monetary losses, including professional fees (such as attorneys, accountants or credit repair services) directly resulting from the data incident.
  • Alternative cash payment: In lieu of ordinary or extraordinary losses, a $40 cash payment.

If the total amount claimed by class members exceeds the available funds, payments may be reduced on a pro rata basis.

How to claim a class action payment

Class members can file a claim online or download, print and complete the PDF claim form and mail it to the settlement administrator. Claims must be submitted or postmarked by Sept. 26, 2025.

What proof or documentation is required to submit a claim?

  • All class members must provide the claim ID located on the notice they received. Online claimants must also provide the PIN located on the same notice. Those who did not receive or misplaced their login credentials may contact the settlement administrator at 844-979-7301 or CV3Settlement@rg2claims.com.
  • For ordinary loss claims, class members must provide receipts, bank statements or other records showing the expense was a result of the data incident.
  • For extraordinary loss claims, class members must provide invoices or statements for professional services, proof of monetary loss and evidence that the loss was more likely than not caused by the data incident.
  • For lost time claims or alternate cash payments, class members simply need to check the appropriate box.

Data breach settlement fund breakdown

When is the CommerceV3 settlement payout date?

Payments for valid and approved claims will begin either 14 days after appeals are resolved or 65 days after final approval.
