Consumers who received a notice from Allied-Locke Industries Inc. about a data security incident that occurred in November 2021 may qualify to claim up to $4,000 from a class action settlement.

Allied-Locke Industries Inc. agreed to settle a class action lawsuit alleging it failed to safeguard personal and health information during a data security incident that took place Nov. 8-14, 2021. The plaintiffs claimed unauthorized third-party actors accessed files containing sensitive information, including:

  • Names
  • Addresses
  • Social Security numbers
  • Dates of birth
  • Financial account details
  • Medical information
  • Health insurance information

Who can file an Allied-Locke claim?

Class members must meet all of the following criteria:

  • They reside in the United States.
  • The Allied-Locke data security incident that occurred between Nov. 8, 2021, and Nov. 14, 2021, compromised their personal information.
  • They received a direct notice from Allied-Locke about the data security incident.

The class includes approximately 2,619 individuals Allied-Locke identified. This group includes current and former employees, as well as any other individuals whose information Allied-Locke stored on its systems and who received a notification.

How much can class members receive?

  • Ordinary losses: Up to $500 for documented out-of-pocket expenses, such as costs for credit reports, credit monitoring, identity theft insurance, bank transactions, postage and gasoline for local travel. Class members can also claim lost time spent dealing with the incident (up to four hours at $20 per hour for a maximum of $80) as part of the $500 cap.
  • Extraordinary losses: Up to $3,500 for documented, actual and unreimbursed monetary losses most likely related to the data breach. This may include unreimbursed losses from identity theft or fraud, professional fees (such as attorneys’ or accountants’ fees), costs associated with freezing or unfreezing credit and other related expenses. The loss must have occurred after Nov. 8, 2021, and not be covered by other reimbursement categories.
  • Credit monitoring: Two years of one-bureau credit monitoring services with at least $1 million in identity theft insurance

How to claim a settlement payment

Class members can submit the online claim form or download, print, complete and mail the PDF claim form to the settlement administrator. The claim deadline is Nov. 19, 2025.

What proof and documentation is required to submit a claim?

  • To file an online claim, class members must provide the CPT ID and passcode found on the settlement notice they received. Those who do not have a CPT ID and/or passcode or cannot locate them should email AlliedLockeSettlement@cptgroup.com.
  • To claim ordinary losses, class members must provide documentation, such as receipts for credit monitoring, bank fees or other related expenses.
  • To claim extraordinary losses, class members must provide proof of actual, unreimbursed monetary loss, such as receipts, bank statements or other evidence, and demonstrate the data incident most likely caused the loss.
  • To claim lost time, class members must provide a brief description of the activities spent dealing with the incident.
  • To claim credit monitoring, class members only need to select this option on the claim form.

When is the Allied-Locke Industries settlement payout date?

The settlement administrator will send credit monitoring codes to valid claimants approximately 45 days after it receives their claim form. It will send payments approximately 45 days after the court approves the settlement and resolves any appeals.

Why did this class action settlement happen?

The class action lawsuit alleged a data security incident that occurred between Nov. 8, 2021, and Nov. 14, 2021, allowed unauthorized third parties to access files containing personal and health information on Allied-Locke’s systems. The plaintiffs claimed Allied-Locke failed to adequately protect this information, resulting in potential harm to those affected.

Allied-Locke denies any wrongdoing but agreed to settle to avoid the cost and uncertainty of further litigation.
