Revere Health experienced a data breach. The medical organization disclosed the data breach to the U.S. Department of Health and Human Services on Aug. 19, 2025. The cybersecurity incident compromised both personally identifiable information (PII) and protected health information (PHI).

The breach was reported as a hacking incident. Exposed information may include names, contact information, dates of birth, Social Security numbers, driver's license or state ID numbers, health insurance information, medical records and payment information.
