
Individuals in the United States whose private information the ConnectOnCall data breach that occurred between Feb. 16, 2024, and May 12, 2024, may have compromised could qualify to submit a claim for up to $5,000 from a class action settlement. The cyberattack impacted more than 900,000 people.
ConnectOnCall.com LLC and Phreesia Inc. agreed to pay $4.95 million to settle a class action lawsuit claiming a cybersecurity incident exposed provider-patient communications and other sensitive data. The complaint alleged negligence, breach of contract, invasion of privacy and unjust enrichment.
Who can file a claim for a data breach payout?
Class members are individuals residing in the United States whose private information the ConnectOnCall data breach that occurred between Feb. 16, 2024, and May 12, 2024, may have impacted.
How much are settlement payments?
Class members have the following benefit options:
- Cash payment A – Documented losses: Class members can claim up to $5,000 for out-of-pocket losses related to the data breach.
- Cash payment B – Alternate cash: Class members who do not submit a documented losses claim can submit a claim to receive a cash payment of up to $75. The settlement administrator will determine the final payment amount by the number of claims filed.
- Dark web and medical data monitoring: All class members can elect to receive two years of CyEx Medical Shield Complete. Services include dark web and medical identity monitoring, real-time alerts and up to $1,000,000 in medical identity theft insurance.
How to claim a class action rebate
To receive a settlement payment, class members can file a claim online or print and complete the PDF claim form and mail it to the settlement administrator.

Required proof and claim information
- To submit a claim online, class members must provide the settlement unique ID and PIN from the settlement notice they received. To file a claim by mail, class members must provide the unique ID.
- To submit a documented losses claim, class members must provide supporting documentation, which may include receipts, bank or credit card statements showing unreimbursed fees or charges, or other proof of fraud or identity theft.

Settlement fund breakdown
The $4,950,000 settlement fund will include:
- Settlement administration costs: Estimated at $490,000

- Dark web and medical monitoring: Cost determined by number of claims filed
- Payments to approved claimants: Remaining settlement funds

When is the ConnectOnCall data breach settlement payout date?
The settlement administrator will issue payments approximately 80 days after the court grants final approval of the settlement.
Why is there a class action settlement?
The class action lawsuit alleged ConnectOnCall discovered an unauthorized party accessed its platform and exfiltrated data, including provider-patient communications. The plaintiffs claimed negligence, breach of contract, invasion of privacy and unjust enrichment.
The defendants deny any wrongdoing but agreed to settle to avoid the uncertainty and cost of further litigation and a possible trial.
