Protected health information may also have been compromised, including medical information and health insurance information.

Glendale Obstetrics & Gynecology is offering affected individuals 12 months of free single-bureau credit monitoring, credit report and credit score services through Cyberscout, a TransUnion company that specializes in fraud assistance and remediation. These services send alerts when changes are made to a credit file.

Affected individuals can enroll by visiting Cyberscout's activation page and entering the unique code included in their notification letter. Enrollment must be completed within 90 days of the date of the letter.

In addition to credit monitoring, the practice is providing proactive fraud assistance to help answer questions or assist in the event someone becomes a victim of fraud. Cyberscout representatives are available to answer questions and help with enrollment by calling 1-833-289-6054.

Representatives can be reached from 5 a.m. to 5 p.m. Pacific time, Monday through Friday, excluding U.S. holidays. This support line will be available for 90 days from the date of the notification letter.

  • Place a fraud alert or credit freeze with Equifax (1-800-525-6285), Experian (1-888-397-3742) or TransUnion (1-833-799-5355) to help prevent unauthorized accounts from being opened.
  • Request free credit reports at AnnualCreditReport.com and review them carefully for any unfamiliar accounts or inquiries.
  • Monitor health insurance statements for any services, claims or charges that were not authorized, since medical information and health insurance information may have been exposed in this breach.
