Who can file a data breach claim?
Consumers are eligible to file a claim if:
- They are a U.S. resident whose personally identifiable information (PII) and/or personal health information (PHI) was exposed in one or both of the data breach incidents.
- Maryville Inc. sent them a notice stating their PII or PHI was potentially compromised in the breaches.
How much is the Maryville settlement payout?
- Out-of-Pocket Losses: Up to $5,000 for out-of-pocket losses related to the data breach
- Compensation for Lost Time: Up to $25 per hour for up to four hours (maximum $100) for time spent mitigating the effects of the data incident. This can be combined with out-of-pocket loss claims but is subject to the $5,000 cap.
- Credit Monitoring: Two years of free credit monitoring from one of the three major credit bureaus
How to claim a Maryville data breach class action rebate
To receive a settlement payment or credit monitoring, class members must file a claim online or download and print the PDF claim form and mail it to the settlement administrator. The claim deadline is July 6, 2025.

Required proof and documentation
- Both online and paper claim forms require the Notice ID included on the official settlement notice or the last four digits of the class member's Social Security Number.
- For out-of-pocket losses, claimants must provide documentation, such as receipts, invoices, unreimbursed losses from fraud or identity theft, professional fees (such as accountants or credit repair), costs to freeze/unfreeze credit or credit monitoring costs incurred after the breach and miscellaneous expenses (notary, postage, copying, mileage, etc.)
