
Patients who accessed Mammoth Hospital’s “Your Medical Record” portal between Aug. 9, 2020, and Aug. 9, 2023, may qualify to claim a $20 cash payment or one year of identity protection services from a class action settlement.
Southern Mono Healthcare District, doing business as Mammoth Hospital, agreed to resolve a class action lawsuit alleging its use of third-party tracking technologies, known as “tracking pixels,” on its website violated certain California privacy laws.
Who can file a Mammoth Hospital claim?
Class members must meet all of the following criteria:
- They are or were Mammoth Hospital patients.
- They used Mammoth Hospital’s web properties to access the “Your Medical Record” portal at https://mammothhospital.org.
- They accessed the portal between Aug. 9, 2020, and Aug. 9, 2023.
How much can class members get?
Class members can claim one or both of the following benefits:
- A one-time $20 cash payment
- One year of CyEx Privacy Shield Pro, an identity protection service that monitors the dark web for personal information and provides alerts and support if fraud is detected
How to claim a settlement benefit
Class members can file a claim online or download, print and complete a PDF claim form and mail or email it to the settlement administrator. The deadline to submit a claim online or have it postmarked by mail is Oct. 14, 2025.

What is required to submit a claim?
To file an online claim, class members must provide the unique ID and PIN located at the top of their email or postcard notice. Those who are unable to locate their ID and PIN should contact the settlement administrator by emailing info@MammothHospitalPixelSettlement.com or calling 833-360-6895.

Class members can receive their $20 cash payment via:

- Venmo
- Zelle
- Virtual prepaid card
- Physical check
Class members who choose CyEx Privacy Shield Pro will receive an enrollment code via email.
Settlement fund breakdown
The settlement fund includes:
- Settlement administration costs: To be determined
- Attorneys' fees, costs and expenses: Up to $380,000
- Service award to class representative: Up to $2,500
- CyEx Privacy Shield Pro costs: To be determined
- Payments to eligible class members: The remainder of the fund

When is the Mammoth Hospital Pixel Settlement payout date?
Payments and benefits will be distributed after the court grants final approval of the settlement and any appeals are resolved.
Why was there a class action settlement?
The class action lawsuit alleged Mammoth Hospital used third-party tracking technologies, known as tracking pixels, on its website in a way that violated California privacy laws.
Mammoth Hospital denies any wrongdoing. The parties agreed to settle the case to avoid the costs, risks and uncertainties of further litigation and to provide benefits to class members without additional delay.
