Consumers who lived in the United States and received a written notice stating the April 2024 Benefit Profiles Inc. data security incident compromised their personal information may be eligible to claim up to $3,000 from a class action settlement.

Benefit Profiles Inc. agreed to settle a class action lawsuit alleging it failed to adequately protect personal information, including Social Security numbers and health insurance information, during a cybersecurity incident in April 2024.

Who can file a claim?

The class includes all living individuals residing in the United States whose personal information the April 2024 data security incident involving Benefit Profiles Inc. potentially compromised and who received written notice of the data security incident.

How much can class members get?

Class members must choose between two main cash payment options:

  • Option 1: Reimbursement for documented losses
    • Ordinary losses (out-of-pocket expenses): Up to $500 for documented expenses, such as fees for credit reports, credit monitoring or freezing or unfreezing credit, costs to replace IDs and postage to contact banks by mail. Proof is required. The losses must have occurred between April 3, 2024, and Jan. 20, 2026.
    • Extraordinary losses (identity theft or fraud): Up to $2,500 for documented losses due to identity theft or fraud that the data security incident most likely caused by. Proof is required. The losses must not already be covered by ordinary losses, and claimants must show they tried to prevent the loss or recover their money (for example, by using insurance). These losses must also have occurred between April 3, 2024, and Jan. 20, 2026.
  • Option 2: Alternative cash payment
    • A one-time $50 cash payment in lieu of any other financial benefits. No documentation is required for this option.

All class members are also eligible to enroll in two years of three-bureau credit monitoring services, which includes $1 million in identity theft protection insurance.

How to claim a class action payment

Class members can file the online claim form or download, print and complete the PDF claim form and mail or email it to the settlement administrator.

Class members may also request a paper claim form by calling the settlement administrator at 833-417-4909 or emailing info@BenefitProfilesDataSettlement.com.

The claim deadline is Jan. 20, 2026.

What proof or documentation is necessary to submit a claim?

  • To file an online claim form, class members must log in with the unique ID and PIN located on the settlement notice they received. Those who are unable to locate their ID and PIN should contact the settlement administrator and provide their full name and mailing address.
  • To file an ordinary losses claim, class members must provide receipts or other proof of out-of-pocket expenses.
  • To file an extraordinary losses claim, class members must provide bank statements or other documentation showing unreimbursed losses due to identity theft or fraud.
  • To claim the alternative cash payment, class members must check the appropriate box on the claim form.

Settlement fund breakdown
