Individuals who received a notice that their private information was compromised in the Apex Global Solutions data breach between June 18, 2024, and July 2, 2024, may be eligible to submit a claim for up to $3,500 and three years of free medical data monitoring from a class action settlement. The breach compromised personal and protected health information belonging to approximately 390,000 current and former client patients.

Apex Global Solutions Inc., a company that provides revenue cycle management for skilled nursing and long-term care facilities, has agreed to pay to settle a class action lawsuit stemming from a cybersecurity incident that allegedly resulted in unauthorized access to sensitive information belonging to client patients. The plaintiffs claimed that the company failed to protect private data, including names, addresses, dates of birth, Social Security numbers, driver’s license numbers, financial account details and health information.

Who can file a claim for a data breach payout?

Class members are individuals residing in the United States who received a notice directly from Apex Global Solutions stating that their private information may have been impacted by the data breach that occurred between June 18, 2024, and July 2, 2024.

How much will the settlement payment be?

Class members can submit to receive one or both of the following benefits:

  • Cash payment for documented losses: Class members can claim up to $3,500 in out-of-pocket losses related to fraud or identity theft traceable to the data breach.
  • Medical data monitoring: All class members can also elect to receive three years of free medical data monitoring, which includes monitoring of healthcare insurance plan IDs, beneficiary identifiers, medical records, national provider identifiers, health savings accounts, high-risk transactions and the dark web, along with $1,000,000 of identity theft insurance, real-time alerts and victim assistance.

How to claim a class action rebate

To claim a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail it to the settlement administrator.

Required proof and claim information

  • Unique ID and PIN from settlement notice required to submit a claim.
  • Documented losses claims require supporting documentation, which may include correspondence, receipts, bank or credit card statements showing unreimbursed charges or fees due to fraud, police report, IRS documents showing falsified tax returns and invoices from professionals such as accountants or attorneys.

When is the Apex data breach payout date?
