Individuals whose private information the data incident Affiliated Dermatologists & Dermatologic Surgeons PA discovered on or about March 5, 2024, potentially compromised may be eligible to claim up to $5,000 from a class action settlement.

Affiliated Dermatologists & Dermatologic Surgeons PA agreed to pay $1,000,000 to resolve a class action lawsuit alleging it failed to adequately protect sensitive personal information, including Social Security numbers, dates of birth, medical treatment details, health insurance claims, driver’s license numbers, passport numbers and other nonpublic personally identifiable or protected health information.

How much can class members get?

The settlement provides several types of benefits for class members, depending on their circumstances and preferences:

  • Cash Payment A – Documented losses: Up to $5,000 for reimbursement of expenses, including bank fees, postage, gasoline for local travel and fees for credit reports, credit monitoring or identity theft insurance products. Class members must provide documentation, such as receipts, phone records or emails.
  • Cash Payment B – Alternate cash: Class members who do not wish to submit documentation or who do not have any documented losses may instead claim a flat $40 payment by submitting a claim form.
  • Credit monitoring: All class members will automatically receive three years of free credit monitoring. They do not need to submit a claim form for this benefit.

If the total amount of valid claims for cash payments exceeds $1,000,000, the settlement administrator will reduce each cash payment on a pro rata (equal share) basis.

Class members can file the online claim form or download, print and complete the PDF claim form and mail it to the settlement administrator.

What proof or documentation is required to submit a claim?

  • To file an online claim, class members must provide the unique ID printed on the settlement notice they received in the mail. Those who did not receive a notice or lost theirs should contact the settlement administrator at 1-877-734-7165 for assistance.
  • To file a claim for documented losses, class members must provide reasonable documentation, such as receipts, phone records or emails, showing their expenses. A third party or the class member themselves must generate or prepare the documentation. Personal certifications or affidavits alone are not sufficient but may be included for context.
  • To file a claim for the alternate cash payment, class members need only select the appropriate option on the claim form.

$1 million settlement fund breakdown

  • Service awards to class representatives: Up to $2,000 each
