What Happened?

On or around Sept. 16, 2025, Heritage Holdings LP, doing business as Heritage Communities, Orchard Pointe and OnCare Health, experienced a network intrusion that impacted a limited number of systems.

On Oct. 9, 2025, the hacking group, "Worldleaks", claimed responsibility for the breach and posted about it on the Tor network, alleging they had leaked corporate data from Heritage Communities. The company quickly secured its network and launched an investigation with third-party forensic experts to determine the scope of the incident.

The investigation discovered that PII and PHI of current and former residents may have been accessed by an unauthorized party.
