Freedom Square of Seminole is a luxury senior living community located in Seminole, Florida. Established in 1984, the community offers a range of care options, including independent living, assisted living, memory care, skilled nursing and rehabilitation.

The community is managed by Life Care Services, a senior living management company. Freedom Square of Seminole provides over 300 independent living residences, nearly 150 assisted living units, a 236-bed skilled nursing facility and 25 memory care suites.

What happened?

Freedom Square of Seminole discovered a data breach on or around Sept. 16, 2024. The incident occurred between Sept. 9, 2024, and Sept. 12, 2024 when unauthorized parties accessed sensitive personal information belonging to residents and possibly others associated with company.

The data breach affected thousands of residents at multiple Life Care Services facilities. At least 5,849 individuals were affected in the Freedom Square of Seminole data breach, including five in Maine and 14 in Massachusetts.

The company notified affected consumers in writing on July 11, 2025, and reported the breach to the Maine, Massachusetts and Vermont attorney general offices in July 2025.
