Freedom Square of Seminole, a luxury senior living community in Seminole, Florida, experienced data breach affecting thousands of individuals. Freedom Square discovered suspicious activity on or around Sept. 16, 2024 and an investigation revealed the data breach occurred between Sept. 9 and Sept. 12, 2024, impacting at least 5,849 people.

An unauthorized party accessed data containing both both personally identifiable information (PII) and protected health information (PHI) of patients and staff. Compromised information includes first and last names, addresses, dates of birth, Social Security numbers, driver’s license or state identification card numbers, passport numbers, states military identification numbers, USClS or alien registration numbers, account usernames and password information, credit card numbers, debit card information, financial account numbers, Medicaid numbers, Medicare numbers, health insurance group and policy numbers, health insurance prescription information, medical record numbers, medical treatment or procedure details, mental or physical diagnosis information, patient account numbers, provider names and locations, and biometric information.

Freedom Square is one of multiple Life Care Services (LCS) senior facilities impacted by the cybersecurity incident. Other LCS communities involved in the data breach include Regency Oaks and Freedom Plaza Senior Living.

Freedom Square of Seminole disclosed the data breach to the Maine, Massachusetts and Vermont Attorney Generals' offices beginning on July 12, 2025. The company also published a Notice of Security Incident on its own website on June 18, 2025.

Freedom Square of Seminole's response

In addition to the required state disclosures, Freedom Square of Seminole began notifying affected individuals by mail on July 11, 2025. The company is also offering free IDX credit monitoring and identity protection services to impacted residents and staff.
