What Happened?

On July 25, 2025, ERMI discovered that an unauthorized individual may have gained access to a limited number of employee email accounts.

After a thorough investigation with external cybersecurity professionals, it was determined that files may have been accessed or removed by the unauthorized individual between Feb. 15, 2025, and Aug. 14, 2025.

The company conducted a detailed review of the potentially impacted data and, on April 17, 2026, determined that some files may have contained personal information.

Information Exposed:

  • Health Records
  • Health Insurance Information
  • Medical Information
  • Names of individual
  • Social Security Numbers
  • Driver’s License numbers
  • Financial Information (e.g. account number, credit or debit card number)
  • Dates of Birth

So far, 9,453 Texas residents were impacted as a result of the breach.
