Lynn Community Health Center recently experienced a data breach affecting approximately 6,200 patients. The cybersecurity incident originated with TriZetto Provider Solutions, a subcontractor of OCHIN EPIC, which provides electronic health record services to the center.

TriZetto Provider Solutions identified a security incident that exposed certain protected health information (PHI) belonging to Lynn Community Health Center patients. Potential information exposed includes names of patients and primary insureds, addresses, dates of birth, Social Security numbers, health insurance member numbers (and in some cases, Medicare beneficiary numbers), health insurer names, information about the primary insured or beneficiary, and other demographic, medical and health insurance information.

After detecting the incident, TriZetto Provider Solutions promptly launched an internal investigation and brought in cybersecurity experts to assess and contain the breach. Law enforcement was also notified, and additional security measures were implemented to help prevent similar incidents in the future.

The community health center posted a data breach notice on its website and has notified impacted individuals by mail.

In response to the breach, Lynn Community Health Center has worked closely with OCHIN EPIC and TriZetto Provider Solutions to ensure transparency with their patients. The center has stated that TriZetto Provider Solutions will notify all affected patients directly and will offer appropriate assistance services.
