Life in Motion Family Wellness Center, operated in Indiana by Kathalene Keller Riney NP, LLC, experienced a data breach affecting thousands of patients. The medical practice disclosed the cybersecurity incident to the U.S. Department of Health in Human Services on Aug. 12, 2025, reporting at least 3,747 individuals affected.
According to the disclosure, the cybersecurity incident compromised both personally identifiable information (PII) and protected health information (PHI). Exposed information may include names, contact information, dates of birth, Social Security numbers, driver's license or state ID numbers, health insurance information, medical records and payment information.
In addition to required state and federal disclosures, Life in Motion Family Wellness Center will notify impacted individuals by mail.
