Current and former patients who received a written notice from that their private information was compromised in the July 2025 Susan B. Allen Memorial Hospital data breach, may be eligible to submit a claim for up to $100 plus credit monitoring from a class action settlement.

Susan B. Allen Memorial Hospital has agreed to pay to settle a class action lawsuit for alleged failures to protect patient data during a targeted cyberattack in July 2025. The incident impacted both PII and PHI belonging to more than 12,000 people.

Who can file a claim for a data breach payout?

Class members are individuals who were sent written notice of the July 2025 Susan B. Allen Memorial Hospital data breach.

How much can class members get?

Class members have the following benefit options:

  • Credit monitoring and identity theft protection: All class members can elect to receive two years of CyEx Medical Shield Complete. Services include $1 million in medical identity theft insurance, access to a fraud resolution agent if needed, monitoring for healthcare insurance ID and medical record number exposure and unauthorized health savings account spending.
  • Reimbursement for out-of-pocket losses: Class members can claim up to $100 for documented out-of-pocket losses traceable to the breach. This may include credit monitoring fees, cost to replace IDs, postage for contacting financial institutions and other losses due to identity theft or fraud.
  • Reimbursement for attested time: Class members can claim up to four hours, at $25 per hour, for time spent dealing with the data breach, a maximum of $100. This includes researching the incident, changing passwords and investigating suspicious activity.

There is an aggregate cap of $100 in cash benefits. The total payment amount a class member can receive is $100.

How to claim a class action rebate

To receive a settlement payment, class members can file a claim online or print and complete the PDF claim form and mail or it to the settlement administrator.

Required proof and claim information

  • To submit an online claim, class members must provide the LoginID and PIN from the settlement notice they received.
  • Out-of-pocket losses claims require supporting documentation, which may include receipts, bank or credit card statements, correspondence and other proof of fraud or identity theft.
  • Attested time claims require a description of how time was spent.

Settlement fund breakdown

The settlement fund will include:

  • Settlement administration costs: At least $15,000
  • Attorneys' fees and costs: Up to $150,000

  • Credit monitoring: Cost determined by number of claims filed.
  • Payments to claimants: Up to $100 each
