Additionally, 2,071 total patients in Lubbock and 666 total patients in Amarillo were impacted, according to the corresponding disclosures filed with the U.S. Department of Health and Human Services.

Both personal identifiable information and protected health information was exposed in this breach including information types such as names, addresses, dates of birth, medical information and health insurance information.
