Excellent Home Care Services, LLC, a Brooklyn-based home health agency, recently experienced a data breach that may have affected individuals in Bronx, Kings, Nassau, New York and Queens counties. The information that was potentially compromised includes a combination of personally identifiable information (PII) and protected health information (PHI).

On Nov. 25, 2025, the company discovered that an employee’s email account was accessed by an unauthorized third party for a brief period. The breach was limited to a single email account, but the company could not determine exactly how many files were viewed.

Specifically, the data exposed may have included full names, addresses, phone numbers, dates of birth, gender, Social Security numbers, Medicare or Medicaid numbers and certain medical information related to patient plans of care, such as diagnoses, medications or plan of care documentation.

On Dec. 17, 2025, the company sent notification letters to all affected individuals and posted a Notice of Data Breach press release online on Dec. 18, 2025.

Excellent Home Care Services' response

After discovering the breach, Excellent Home Care Services immediately secured the affected email account and began working with external cybersecurity experts to investigate the incident. The company put additional safeguards in place, including enhanced access controls, geographic restrictions and improved monitoring tools. Excellent Home Care Services is also offering free identity monitoring services to impacted individuals.
