In September 2024, Dr. Doug's Pediatric Dentistry, a pediatric dental practice based in Logan, Utah discovered suspicious activity involving one of its internal email accounts. An investigation was launched and determined that personally identifiable information (PII) and protected health information (PHI) may have been compromised in the cybersecurity incident.

A review was completed in June 2025, and it was determined that exposed information may include names, dates of birth, driver's license information, Social Security number, diagnosis or dental treatment information, dental insurance information and Medicaid information. Dr. Doug's Pediatric Dentistry began notifying affected patients by mail and published a Notice of Data Security Incident on its website on Aug. 14, 2025.

The total number of impacted current and former patients has not been released. The nature of the data breach is severe, and could lead to identity theft or misuse of medical information.

Dr. Doug's Pediatric Dentistry's response

In addition to required state and federal disclosures, Dr. Doug's Pediatric Dentistry established a toll-free call center to answer questions about the incident at 833-594-5308, available Monday through Friday from 7:00 am to 7:00 pm Mountain Time.
