
360 Dental PC, a dental practice based in Northeast Philadelphia, recently experienced a data breach that impacted 11,273 individuals in the United States. On Nov. 16, 2025, the organization discovered that an unauthorized party had accessed its internal server, locking files and making them temporarily inaccessible.
The exposed information includes both personally identifiable information (PII) and protected health information (PHI): patient names, Social Security numbers, dates of birth, addresses, telephone numbers, email addresses, patient account or chart numbers, dental and clinical records (such as treatment history, clinical notes, X-rays and diagnostic information), insurance provider and member ID, appointment information and emergency contacts.
The breach was reported to the U.S. Department of Health and Human Services on Jan. 15, 2026. The company has also posted a notice to consumers.
The company has taken several steps to strengthen its cybersecurity posture: replacing affected computers, rebuilding the server, updating all software, implementing additional firewalls, installing antivirus protection, enabling multi-factor authentication and restricting access to VPN-only connections.
Regular secure offsite backups and ongoing monitoring have also been put in place to help prevent future incidents.
