The breach was reported to the U.S. Department of Health and Human Services on Jan. 15, 2026. According to official disclosures, the total number of people affected in the United States is 11,273.

- Addresses
- Telephone numbers
- Emails
- Patient account or chart numbers
- Dental and clinical records (including treatment history, clinical notes, X-rays and diagnostic information)
- Insurance providers and member IDs
- Appointment information
