Individuals who received a notice from Oglethorpe Inc. stating a June 2025 data breach may have affected their private information could be eligible to claim up to $2,500 plus medical data monitoring from a class action settlement.

Oglethorpe Inc. agreed to settle a class action lawsuit claiming a targeted cyberattack compromised sensitive personal information, including names, Social Security numbers, driver’s license or state identification numbers, and medical details.

Who can file a claim?

Class members must meet all of the following criteria:

  • They are a living individual residing in the United States.
  • They received a notice from Oglethorpe Inc. indicating the June 2025 data incident may have impacted their private information.

How much can class members get?

Class members are eligible for several types of benefits under the settlement.

  • Documented losses: Up to $2,500 for actual, documented out-of-pocket losses that occurred between June 1, 2025, and July 8, 2026, and resulted from the data incident. Eligible expenses include losses from identity theft or fraud, fees for credit reports or monitoring, costs to replace IDs and postage for contacting banks. Class members must provide proof, such as bank statements or receipts, to support their claim.
  • Alternate cash payment: As an alternative to claiming documented losses, class members may claim a one-time $75 cash payment. No documentation is required for this option.
  • Medical data monitoring: All class members automatically receive one year of CyEx Medical Shield Complete Medical Data Monitoring.

The settlement caps all cash payments at $350,000. If the total amount all class members claim exceeds this cap, the settlement administrator will reduce payments proportionally so the total does not exceed $350,000.

How to claim a settlement payment

Eligible class members can submit an online claim form or download, print and complete a PDF claim form and mail it to the settlement administrator. They may also request a paper copy from the settlement administrator by email or phone. The claim deadline is July 8, 2026.

What proof or documentation is required to submit a claim?

  • To file an online claim form, class members must log in with the unique ID and PIN from the settlement notice they received. Those who are unable to locate their ID and PIN should contact the settlement administrator by emailing info@Oglethorpe2025DataIncident.com and providing their full name and mailing address.
  • To file a claim for the $2,500 documented loss payment, class members must provide proof, such as receipts, bank statements or other documentation, showing the data breach caused the expenses. Self-prepared notes alone are not sufficient, but class members may use them to supplement their proof.
  • To file a claim for the $75 alternate cash payment or medical data monitoring, class members do not need to provide documentation.
