Who can file a claim?
Class members must meet the following criteria:
- They resided in California when they visited NorthBay's website or patient portal during the period from Nov. 29, 2020, to May 14, 2024.
- Tracking tools on NorthBay's websites or patient portal may have disclosed their personal or health information to a third party without authorization or consent.
How much is the class action settlement payout?
Class members can claim one or more of the following benefits:
- Cash payment: Class members who submit a valid and timely claim form can receive a one-time payment of $15.
- Privacy protection service: All class members will automatically receive a one-year subscription to CyEx Privacy Shield Pro at no cost. The settlement administrator will send enrollment codes by email or mail.
How to claim a NorthBay Healthcare class action settlement payment
To receive the $15 cash payment, class members must file a claim online or download, print and mail or email a completed PDF claim form to the settlement administrator. Class members may also request a paper claim form by calling the toll-free number or emailing the settlement administrator. The claim deadline is March 12, 2026.
