Individuals qualify for the settlement if the April 4, 2023, Morris Hospital data incident potentially compromised their personal information and they received a notification regarding the breach.
How much is the data breach payout?
- Reimbursement for out-of-pocket losses: Up to $5,000 per person for documented expenses related to the data incident, such as:
- Unreimbursed fraud
- Credit monitoring or fraud resolution services purchased after the incident
- Credit report costs
- Bank or financial institution charges
- Other reasonable losses resulting from the data incident
- Alternative cash payment: Estimated at $100. The actual amount depends on the number of valid claims and funds remaining after other expenses.
- Free credit/medical monitoring and identity theft protection services (CyEx Medical Shield Total): Up to 24 months of real-time credit file monitoring at all three major bureaus; $1,000,000 in identity theft insurance (no deductible); dark web, medical record, health insurance and health savings account monitoring; and access to fraud resolution agents.
Class members must choose between the alternative cash payment and reimbursement for out-of-pocket losses; they cannot claim both.
How to claim a settlement payment
Class members can file an online claim form or download, print, complete and mail the PDF claim form to the settlement administrator. The claim deadline is Oct. 28, 2025.

What proof or documentation is required to submit a claim?
- All class members must provide the unique ID located on the settlement notice they received. Online claimants must also provide the PIN from the same notice. Those who are unable to locate their ID and PIN should contact the settlement administrator by emailing info@MorrisHospitalSettlement.com or calling 866-603-5601 and providing your full name and mailing address.
- For reimbursement of out-of-pocket losses, class members must provide documentation, such as receipts, bank statements or bills showing the loss and a statement explaining how it relates to the data breach.
- For alternative cash payment or credit monitoring, class members need only select the appropriate option.

When is the Morris Hospital data breach settlement payout date?
The settlement administrator will distribute payments and credit monitoring codes approximately 31 days after the court grants final approval of the settlement and resolves any appeals.
