McKenzie Health System Data Breach Class Action Settlement

Current and former patients who received a notice that the April 2025 cyberattack on McKenzie Memorial Hospital, d/b/a McKenzie Health System, compromised their private information may be eligible to claim up to $4,000 from a class action settlement. The data breach impacted more than 58,000 individuals.

McKenzie Memorial Hospital, d/b/a McKenzie Health System, agreed to pay to settle a class action lawsuit alleging it failed to protect patient and consumer data during a targeted cyberattack. The data incident compromised files containing personally identifiable and protected health information belonging to patients, which included dates of birth, Social Security numbers and medical records.

Who can file a claim for a data breach payout?

Class members are individuals in the United States whose private information the April 2025 McKenzie Memorial Hospital data breach potentially compromised, including those who received a notice about the incident.

How much are settlement payments?

Class members have the following benefit options:

  • Cash payment A for out-of-pocket losses: Class members can claim up to $4,000 for documented out-of-pocket losses that occurred between April 14, 2025, and Aug. 24, 2026, and are traceable to the data breach. Eligible expenses include monetary losses from identity theft or fraud; fees for credit reports, credit monitoring or freezing/unfreezing credit; costs to replace IDs; and postage for contacting financial institutions.
  • Alternative cash payment B: Class members who do not submit a documented losses claim can submit a claim to receive a one-time $50 payment.
  • Credit monitoring: All class members can elect to receive two years of CyEx Financial Shield Complete credit monitoring. Services include monitoring for fraud or identity theft, monitoring of personal information associated with high-risk transactions, alerts for unauthorized financial transactions, $1 million in financial fraud insurance and access to fraud resolution agents if needed.

How to claim a class action rebate

To receive a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

  • Online claims require the login ID and PIN from the settlement notice the class member received.
  • Out-of-pocket losses claims require supporting documentation, which may include receipts, bank or credit card statements showing unreimbursed fees or fraudulent charges, or evidence of identity theft or fraud.

When is the McKenzie Health System settlement payout date?

The settlement administrator will issue payments to approved claimants after it completes claim processing and the court grants final approval of the settlement.

Why is there a class action settlement?

This class action lawsuit claimed a targeted cyberattack in April 2025 allowed unauthorized access to McKenzie Health System’s computer systems. The files accessed during the incident reportedly contained sensitive personal and medical information belonging current and former patients. The plaintiffs alleged McKenzie failed to adequately protect this data.

McKenzie Health denies the allegations but agreed to settle to avoid the expense and risk of continued litigation.
