
Current and former patients and other individuals who received a letter from General Physician P.C. informing them a data breach that occurred between April 6, 2024, and June 12, 2024, may have accessed their private information may qualify to submit a claim for up to $5,000 and credit and medical monitoring from a class action settlement. The incident impacted an estimated 490,210 individuals.
General Physician agreed to pay $2.5 million to settle a class action lawsuit related to a cybersecurity incident that allegedly exposed sensitive personal and medical information belonging to current and former patients. PII and PHI compromised in breach included names, addresses, Social Security numbers, dates of birth, medical treatment and record information and financial account details.
Who can file a claim for a data breach payout?
Class members are individuals in the United States whose private information the data breach General Physician reported in October 2024 potentially accessed and/or acquired.
How much will the settlement payment be?
Class members have the following benefit options:
- Cash payment A – Documented losses: Class members can claim up to $5,000 for documented losses traceable to the data incident.
- Cash payment B – Alternate cash: Class members who do not submit a documented losses claim can submit a claim to receive a pro rata cash payment estimated at $60 but no more than $599. The settlement administrator will determine the final payment amount by the total number of claims filed.
- Credit and medical records monitoring: All class members can elect to receive two years of one-bureau credit and medical records monitoring valued at $179.40 per year. Services include real-time alerts and up to $1,000,000 in medical identity theft insurance coverage.
How to claim a class action rebate
To receive a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

Required proof and claim information

When is the General Physician PC data breach settlement payout date?
