
Individuals whom Consulting Radiologists Ltd. notified that an April 2024 data incident may have impacted their private information could be eligible to claim up to $5,000 plus a cash payment adn credit monitoring from a class action settlement.
Consulting Radiologists Ltd. agreed to pay $2,200,000 to resolve a class action lawsuit alleging it failed to implement and maintain reasonable security measures to protect private information, resulting in unauthorized access by a third party.
Who can file a claim?
Class members must meet the following criteria:
- They are an individual residing in the United States whose private information the Consulting Radiologists data incident disclosed in April 2024 potentially exposed.
- They received a notice from Consulting Radiologists Ltd. regarding the data incident or otherwise meet the settlement class definition.
How much can class members get?
Class members may be eligible for the following benefits:
- Expense reimbursement: Up to $5,000 for documented, unreimbursed out-of-pocket losses that are fairly traceable to the data incident. Class members must provide supporting documentation to qualify.
- Examples of reimbursable losses include unreimbursed costs resulting from identity theft or fraud, falsified tax returns, credit monitoring costs, unreimbursed bank fees, postage and gasoline for local travel related to mitigating the effects of the data incident.
- Eligible losses must have occurred on or after Feb. 1, 2024, and before March 2, 2026.
- Cash payment: All class members may claim a cash payment.
- Class members whose Social Security numbers the data incident potentially accessed may receive a cash payment of up to $125.
- Class members whose Social Security numbers the data incident did not access may receive a cash payment of up to $50.
- Credit monitoring: Two years of CyEx Identity Defense Complete, a single-bureau credit monitoring and identity protection service that includes $1 million in identity theft insurance.
- Class members may claim this benefit in addition to expense reimbursement and/or a cash payment.
The settlement administrator will distribute the fund in the following order: settlement administration costs, attorneys’ fees and costs, service awards to class representatives, purchase of credit monitoring services and then payments to class members.
If the number of valid claims exceeds the available funds, the settlement administrator will reduce all payments on a pro rata basis.
How to claim a Consulting Radiologists data breach payment
Class members can submit the online claim form or download, print and complete the PDF claim form and mail it to the settlement administrator. The claim deadline is March 2, 2026.

Proof or documentation required to submit a claim
- For online or mailed claims, claimants must provide the claim ID from the notice they received, as well as their contact information.
- For out-of-pocket expense reimbursement, claimants must provide reasonable third-party documentation of actual, unreimbursed losses that are fairly traceable to the data incident. Self-prepared documents alone are not sufficient.
- For cash payments and credit monitoring, claimants do not need to submit documentation of losses. They must attest to their eligibility and submit the claim form.
