The breach occurred between Dec. 13, 2024, and Dec. 16, 2024 when unauthorized parties accessed email and SharePoint accounts.

The data breach compromised both personally identifiable information (PII) and protected health information (PHI). Synergy Radiation Oncology Medical Group disclosed the cybersecurity incident to the California Attorney General's office on July 15, 2025 and is notifying affected patients by mail.
