Manhattan Retirement Foundation dba Meadowlark Hills, a nonprofit retirement community and nursing facility in Manhattan, Kansas, recently experienced a data breach impacting thousands of individuals across the United States. The breach affected a total of 14,442 people nationwide, including one Massachusetts resident.

The incident was discovered after unauthorized access to the organization’s network occurred between July 12 and July 21, 2025. Following a comprehensive investigation that concluded Jan. 28, 2026, it was determined that sensitive personal and health information was exfiltrated from their systems.

The exposed data includes both personally identifiable information (PII) and protected health information (PHI): full names, Social Security numbers, dates of birth, driver’s license or state identification numbers, other government identification numbers, financial account details, credit or debit card information, medical information and health insurance policy information.

The company filed disclosures with the Massachusetts Office of Consumer Affairs and Business Regulation and the U.S. Department of Health and Human Services.

The organization began sending written notifications to affected individuals on or about Feb. 26, 2026, as required by law. Additionally, a notice of the incident was posted to their website.

Meadowlark Hills' response

To support those affected, Meadowlark Hills is offering complimentary single-bureau credit monitoring, credit reports and credit score services for a period of time to individuals whose Social Security numbers were involved. These services are provided at no cost and include alerts for any changes to credit files.

Additionally, proactive fraud assistance and remediation services are available through Cyberscout, a TransUnion company.

Affected individuals are encouraged to take several precautionary steps:

  • Enroll in the complimentary credit monitoring service provided
  • Place a fraud alert on credit files by contacting any of the three major credit bureaus
  • Consider placing a security freeze on credit files to prevent new accounts from being opened in their name
  • Obtain and review free annual credit reports for discrepancies or unauthorized activity
  • Remain vigilant for signs of identity theft or fraud in financial and medical statements

The company has also provided a dedicated, confidential toll-free response line at 1-844-937-3566, available Monday through Friday, 8 am to 8 pm Eastern time, for individuals seeking more information or assistance.
