What Happened?

Proxycare discovered on Aug. 22, 2025, that certain systems within its network had been affected by a cybersecurity incident. Upon learning of the issue, the company began an investigation with the help of third-party cybersecurity professionals.

The investigation aimed to determine the extent of the activity and whether personal health information was accessed or acquired by an unauthorized party.

After a thorough forensic investigation and manual document review, Proxycare determined on Jan. 29, 2026, that files potentially accessed and/or acquired by a third party contained personal health information.

The types of consumer information that were exposed in this breach include:

  • Social Security numbers

In Massachusetts, 125 individuals were reported as affected.

The company began notifying affected individuals on March 23, 2026.
