What Happened?
In early February 2025, Episource discovered unusual activity in its computer systems. An investigation determined that a cybercriminal had accessed and copied data from Episource’s systems between January 27, 2025, and February 6, 2025.
On April 23, 2025, Episource posted a Notice of Data Breach on a dedicated response website and began notifying its customers with details on which individuals may have been impacted by the breach. The Texas Attorney General was notified on June 10, 2025, with at least 24,259 Texans affected by the cybersecurity incident.

- Date of birth
- Social Security number
- Medical information including medical record numbers, doctors, diagnoses, medicines, test results, images, care, and treatment
- Health insurance information including health plans/policies, insurance companies, member/group ID numbers, and Medicaid and Medicare government payor ID numbers
