The breach was first detected as early as Jan. 22, 2025. Law enforcement requested that notification to patients and hospital customers be delayed so as not to interfere with their investigation.

Your Rights and Next Steps

If affected by this breach, individuals have several important rights and options. First, it is recommended to carefully review statements from healthcare providers, insurance companies and financial institutions for any suspicious activity. If questionable charges appear, report them immediately to the relevant provider.

Individuals are entitled to a free credit report every 12 months from each of the three nationwide credit reporting agencies. If fraudulent activity is found, request that the credit reporting agency delete the information.

Affected individuals can also place a fraud alert or a security freeze on their credit reports at no cost. A fraud alert can make it harder for someone to open new credit in their name, while a security freeze restricts access to their credit report entirely. Contact information for the major credit bureaus is included in the official notice.

Additionally, the vendor is offering identity protection and credit monitoring services for two years. Those who did not receive a notification letter but believe they may be impacted can call 833-918-1127 and use engagement number B156918 for assistance.
