







Center for Urologic Care of Berks County recently experienced a data breach that affected 543 individuals. The incident occurred when an unauthorized party gained access to some of the company’s IT systems between Sept. 24, 2025 and Oct. 13, 2025. Between Sept. 28, 2025 and Nov. 6, 2025, the company determined that personally identifiable information (PII) and protected health information (PHI) of at least 543 patients had been compromised.
According to the notice of data privacy incident posted on the company's website, the types of data exposed included patients’ names, Social Security numbers, and sensitive medical information such as the names of doctors, diagnoses, prescribed medicines, test results, medical images, and details about care and treatment. This combination of PII and PHI increases the potential severity of the breach, as it could lead to identity theft or misuse of private medical data.
The breach was reported to the U.S. Department of Health and Human Services on Nov. 26, 2025.
After discovering the breach, Center for Urologic Care of Berks County immediately secured its systems and began working with third-party forensic experts to investigate the incident. Law enforcement was also notified. The company has since implemented additional safeguards and technical security measures to strengthen its systems and prevent similar incidents in the future.
For impacted individuals, the company is offering complimentary identity monitoring services. Affected patients are encouraged to review statements from their healthcare providers for any unfamiliar charges and to contact their provider if they notice anything suspicious.
