Boley Centers, Inc. experienced a major data breach affecting both personally identifiable information (PII) and protected health information (PHI) of an unknown number of individuals. Solutions for Administrative Services, Inc. (SAS), the vendor managing Boley Centers' network discovered the cybersecurity incident on June 11, 2025.

SAS launched an investigation and determined the data breach may have exposed names, addresses, dates of birth, Social Security numbers and health insurance information. Boley Centers published a data security incident notice on its website in July 2025.

The breach’s severity is heightened by the fact that it involves both PII and PHI, which are often targeted for financial fraud and medical identity theft.

Boley Centers' response

In addition to required state and federal disclosures, Boley Centers will notify impacted individuals directly. A toll-free response line has been established for further questions and additional information about the data breach at 877-495-0947, Monday through Friday, 9:00 AM – 9:00 PM Eastern time.

If you receive a notice from Boley Centers or Solutions for Administrative Services, Inc. (SAS) about this breach, you may want to:

  • Sign up for free credit monitoring services, if offered.
  • Monitor your credit reports and financial accounts for any unusual activity.
  • Be alert for phishing emails or phone calls that may use your exposed information.
  • Consider placing a fraud alert or credit freeze with major credit bureaus.
