Current and former patients and employees whose private information the September 2023 Youth and Shelter Services data breach potentially compromised may qualify to submit a claim for a cash payment of up to $2,500 from a class action settlement. The cybersecurity incident compromised approximately 20,000 individuals.

Youth and Shelter Services Inc. agreed to settle a class action lawsuit related to a cybersecurity incident in September 2023. The plaintiff claimed unauthorized parties accessed files containing sensitive personal data, such as names, dates of birth, Social Security numbers, driver’s license numbers, financial account information, medical and health insurance details, billing and claims information, passport numbers and other government-issued identification numbers.

Who can file a claim for a data breach payout?

Class members are all living individuals in the United States whose private information the September 2023 data breach potentially compromised, including anyone who received a notice of the incident from Youth and Shelter Services.

How much are settlement payments?

Class members can claim one of the following benefit options:

  • Cash payment A - documented losses: Class members can claim up to $2,500 in documented out-of-pocket losses traceable to the data breach. Eligible losses include fees for credit reports, credit monitoring or freezing/unfreezing credit; costs to replace ID documents; monetary losses due to identity theft or fraud; and postage for contacting financial institutions.
  • Cash payment B - alternate cash payment: Class members who do not submit a documented losses claim can submit a claim to receive a one-time cash payment of $50.

How to claim a class action rebate

To receive a settlement payment, class members can file a claim online or print a PDF claim form to complete and mail to the settlement administrator.

Required claim information and proof

  • All class members must provide the unique ID from the settlement notice they received. Online claimants must also provide the PIN from the same notice.
  • Documented losses claims require supporting documentation, which may include receipts, bank or credit card statements showing unreimbursed fees or fraudulent charges, or other proof expenses or losses incurred due to identity theft or fraud.

Settlement fund breakdown

The settlement fund will include:

  • Settlement administration costs: To be determined
  • Attorneys' fees and costs: Up to $200,000
  • Service award to class representative: $2,500

When is the Youth and Shelter Services data breach payout date?
