Current and former patients and other individuals who received a notice that a data breach at Western Montana Clinic personal health information may have affected their information could qualify to submit a claim for up to $5,000 from a class action settlement. Class members have the following benefit options:

  • Reimbursement for documented out-of-pocket losses: Class members can claim up to $5,000 for documented expenses traceable to the data incident.
  • Reimbursement for lost time: Class members can claim up to three hours at $20 per hour for time spent responding to the data breach ($60 maximum).
  • Medical data monitoring: All class members can elect to receive one year of CyEx Medical Shield Pro monitoring.

To receive a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

Required proof and claim information

  • All class members must provide the notice ID from the settlement notice they received. Online claimants must also provide the confirmation code from the same notice.
  • Lost time claims required a description of time spent.
  • Documented out-of-pocket losses require supporting documentation, which may include receipts, invoices, bank or credit card statements showing unreimbursed fees or fraudulent charges, and other proof of identity theft or fraud.
