
Consumers who received a notice of a cybersecurity event from Precision Imaging Centers on or around June 22, 2023, may be eligible to claim up to $5,000 plus credit monitoring from a class action settlement.
Precision Imaging Centers, a health care provider based in Jacksonville, Florida, agreed to settle a class action lawsuit alleging it failed to adequately protect the personal information of its current and former patients and other individuals, leading to a cybersecurity incident on or around Nov. 2, 2022. During this incident, an unauthorized actor accessed sensitive data, including:
- Names
- Addresses
- Social Security numbers
- Driver’s license or government-issued identification numbers
- Health insurance details
- Medical conditions and diagnoses
- Dates of birth
Who are the class members?
Individuals are considered class members and may file a claim if:
- They are a U.S. resident who received written notification from Precision Imaging Centers on or around June 22, 2023, regarding the cybersecurity event that occurred on or around Nov. 2, 2022.
- Precision Imaging Centers may have sent the notification by direct written notice (such as a letter or email) or substitute notice (such as a public notice if direct mail was undeliverable).
- The data breach may have exposed their personal information to unauthorized third parties.
How much is the data breach payout?
There are two categories for reimbursement:
- Ordinary expense reimbursement: Up to $500 per individual for out-of-pocket losses and/or up to four hours of lost time at $20 per hour (maximum $80) related to the breach. Eligible expenses include costs for accessing or freezing/unfreezing credit reports, notary, postage, copying, mileage and credit monitoring or other mitigative costs incurred on or after Nov. 2, 2022.
- Extraordinary expense reimbursement: Up to $5,000 per individual for unreimbursed costs related to the breach, such as losses from identity theft or fraud, falsified tax returns or other misuse of personal information. This category also allows for up to eight hours of lost time at $20 per hour (maximum $160).
All claims are subject to an aggregate cap of $200,000. If the total of all valid claims exceeds this amount, the settlement administrator will reduce each claim proportionally (pro rata).
Class members may also claim free identity protection and credit monitoring services for two years.
How to claim a settlement payment
Class members can file the online claim form or download a personalized claim form from the same link, print it, complete it and mail it to the settlement administrator. Alternatively, they can request a paper claim form by calling the settlement administrator. The claim deadline is Jan. 31, 2026.

What proof or documentation is required to submit a claim?
- All claimants must provide either the unique identifier from their settlement notice or the name and address they provided to Precision Imaging Centers for patient purposes.
- For expense reimbursement, claimants must provide supporting documentation, such as receipts, bank or credit card statements (with unrelated transactions and all but the first four and last four digits of account numbers redacted) or other proof of incurred fees or charges.
- For lost time, claimants must provide an attestation and a brief description of the actions taken and time spent.
- For credit monitoring, claimants need only select the appropriate box.
Payout options
Claimants may receive payment by check or electronic payment, depending on their claim form selection.
Settlement fund breakdown
The settlement fund includes:
- Settlement administration costs: To be determined
- Attorneys' fees and expenses: Up to $188,000
- Service awards to class representatives: Up to $2,000 each for six representatives ($12,000)
- Credit monitoring costs: Dependent on number of valid claims
- Payments to eligible class members: Up to $200,000

When is the Precision Imaging Centers data breach settlement payout date?
The settlement administrator will issue payments within 60 days of the the date the court grants final approval to the settlement or within 30 days of the date the settlement administrator approves the claim.
Why is there a class action settlement?
The class action lawsuit alleged a cybersecurity event on or around Nov. 2, 2022, allowed an unauthorized individual to access Precision Imaging Centers' network and acquire the sensitive personal information of current and former patients and other individuals.
The plaintiffs alleged Precision Imaging Centers failed to take appropriate care to protect this information. Precision Imaging Centers denies any wrongdoing but agreed to settle to avoid the expense and burden of continued litigation.
