Individuals who received notice that their private information may have been compromised in the Minnesota Orthodontics and Dentofacial Orthopedics data breach that occurred on or around February 2025 were eligible to claim up to $25 or two years of free credit monitoring from a class action settlement.

Minnesota Orthodontics and Dentofacial Orthopedics agreed to settle a class action lawsuit for alleged failure to protect private information during a cybersecurity incident that may have exposed the data of nearly 50,000 current or former employees and patients.

Who was eligible to file a claim?

To have been eligible to file a claim, individuals must have met all of the following criteria:

  • Be a resident of the United States
  • Had their private information potentially compromised in the Minnesota Orthodontics and Dentofacial Orthopedics data incident that occurred in or around February 2025
  • Received notice of the data incident from the defendant or the settlement administrator

How much did class members get?

Class members were eligible to claim either two years of free credit monitoring and identity theft protection services through IDX or a $25 alternative cash payment in lieu of credit monitoring services.

Was proof/documentation required to submit a claim?

Claimants did not need to provide additional proof or documentation to submit a claim. The settlement administrator identified eligible class members.

Payout options

Claimants who opted for the $25 cash payment and filed online could choose an electronic payment method. Those who claimed the $25 cash payment and filed by mail will receive a physical check. Credit monitoring claimants will receive activation codes by mail or email.

Settlement fund

The settlement fund will include:

  • Settlement administration costs: Amount not specified in the notice
  • Attorneys' fees, costs, and expenses: Up to $135,000
  • Service award to class representative: Up to $2,000
  • Payments to approved class members: Up to $50,000 for cash claims; unlimited for credit monitoring (as the cost is borne by the defendant)
