
Individuals in the United States whose private and protected health information the July 2025 cyberattack on Coos County Family Health Services compromised may be eligible to submit a claim for up to $5,000 plus two years of medical identity theft monitoring from a class action settlement.
Coos County Family Health Services Inc. agreed to pay $750,000 to settle a class action lawsuit alleging it failed to protect private information during a data breach that exposed sensitive data, including names, Social Security numbers, dates of birth, contact information, addresses and medical information. The data breach impacted an estimated 40,250 current and former patients.
Who can file a claim for a data breach payout?
Class members are current and former patients residing in the United States whose private information the July 9, 2025, Coos County Family Health data breach may have been compromised.
How much is the settlement payment?
Class members have the following benefit options:
- Cash Payment A – Documented losses: Class members can claim up to $5,000 for documented monetary losses traceable to the data breach. This includes unreimbursed losses from fraud or identity theft, professional fees, credit monitoring costs and related expenses.
- Cash Payment B – Alternate cash payment: Class members who do not submit a documented losses claim can submit a claim to receive an estimated $100 cash payment. The settlement administrator will determine the final payment amount by the total number of claims filed.
- Medical monitoring: All class members can elect to receive two years of medical identity theft monitoring, which includes credit monitoring and up to $1,000,000 in medical identity theft insurance.

To receive a settlement payment, class members can file a claim online or download and print the PDF claim form.pdf) to complete and mail to the settlement administrator.

Class members must submit claims online or postmark them by April 14, 2026.
Required claim information and proof
- To submit a claim online, class members must provide the class Member ID from their settlement notice.
- To submit a documented losses claim, class members must provide receipts, invoices for professional fees, bank or credit card statements showing fees and unreimbursed charges due to fraud or any other documentation showing proof of fraud or identity theft.

When is the Coos County Family Health Services settlement payout date?
The settlement administrator will issue payments and medical monitoring codes to approved claimants approximately 85 days after the court grants final approval of the settlement.
Why is there a class action settlement?
This class action lawsuit alleged a cyberattack in July 2025 compromised the private information of tens of thousands of patients of Coos County Family Health Services Inc. The plaintiffs claimed the company failed to adequately protect sensitive data.
Coos County Family Health denies the allegations but agreed to settle to avoid the expense and uncertainty of further litigation.
