
Individuals whose private information the March 17, 2025, data breach at Compassion Health Care Inc. may have impacted could be eligible to claim up to $5,000 from a class action settlement.
Compassion Health Care Inc. agreed to pay up to $600,000 to resolve a class action lawsuit alleging its deficient data security practices led to a cyberattack that exposed sensitive personal information.
Who can file a claim?
The settlement class includes all living individuals in the United States whose private information the data incident Compassion Health Care Inc. discovered on or around March 17, 2025, impacted. There are approximately 23,600 class members.
Class members must have received a notice stating the data breach may have affected their private information.
Those who believe they are a class member but did not receive a unique ID can contact the settlement administrator at CHCDataSettlement@cptgroup.com or by phone at 1-888-271-0945 to verify their eligibility and obtain their unique ID.
The compromised information potentially includes names, addresses, phone numbers, dates of birth, Social Security numbers, driver’s license numbers, health insurance information, claims information and clinical or diagnostic information.
How much can class members receive?
Class members can claim one of two cash payment options and may also elect to receive two years of credit monitoring.
- Cash Payment A – Documented losses: Up to $5,000 per person for documented losses the data incident caused that another source did not already reimburse. This includes reimbursement for time spent dealing with the incident at $25 per hour for up to four hours (maximum $100).
- Cash Payment B – Alternate cash: A $40 cash payment as an alternative to claiming documented losses. No documentation is required for this option.
- Medical data monitoring: Two years of credit monitoring, which includes medical data monitoring and $1,000,000 in identity theft insurance.
If the total amount of valid claims and other settlement costs exceeds the $600,000 settlement cap, the settlement administrator will reduce payments on a pro rata basis.
How to claim a settlement payment
Class member can file the online claim form or download, print and complete the PDF claim form and mail it to the settlement administrator. The claim deadline is Feb. 23, 2026.

Required information and documentation
- To file an online claim, class members must provide the CPT ID and passcode located on the settlement notice they received. Those filling out the paper claim form only need the CPT ID.
- To file a claim for Cash Payment A (documented losses), class members must provide documentation, including credit card statements, bank statements, invoices, telephone records or receipts. Self-prepared documents alone are not sufficient, but class members can use them to clarify other documentation.
- To file a claim for Cash Payment B (alternate cash), class members only need to select the appropriate option on the claim form.

$600,000 settlement fund breakdown
The $600,000 settlement fund includes:
- Settlement administration costs: To be determined
- Attorneys’ fees and costs: Up to $200,000
- Service awards to class representatives: Up to $1,500 each for three representatives ($4,500 total)
- Medical data monitoring costs: Dependent on number of valid claims
- Payments to eligible class members: The remainder of the fund

When is the Compassion Health Care Inc. settlement payout date?
The settlement administrator will issue payments after the court resolves any appeals and grants final approval to the settlement.
Why is there a class action settlement?
The class action lawsuit alleged a cybercriminal gained unauthorized access to Compassion Health Care Inc.’s computer systems, exposing sensitive personal information. The plaintiffs alleged the company failed to adequately protect this information.
Compassion Health Care Inc. denies any wrongdoing but agreed to settle to avoid the costs and risks of litigation and to provide compensation to those affected.
