Current and former patients who logged into the Boone Health patient portal between April 1, 2021, and Dec. 5, 2002, may qualify to submit a claim for a $20 cash payment and privacy monitoring from a class action settlement.
Boone Health Inc., doing business as Boone Health, and CH Allied Services, doing business as Boone Health and Boone Hospital Center, agreed to settle a class action lawsuit alleging the use of third-party website technologies on its public website resulted in the transmission of certain patient information to third parties.
Who can file a claim?
Class members are current or former patients of Boone Health who logged into the patient portal at least once between April 1, 2021, and Dec. 5, 2022, and resided in Missouri during the qualifying period.
How much are settlement payments?
Cash payment: Eligible class members who submit a valid claim can receive a cash payment of $20.
Privacy Shield Pro: All class members will automatically receive a code to enroll for one year of Privacy Shield Pro, a digital privacy monitoring and identity protection service. A claim form is not required to receive this benefit.
How to claim a settlement payment
To claim a settlement payment, class members can submit the online claim form or print and complete the PDF claim form and mail it to the settlement administrator.

Settlement fund breakdown

When is the Boone Health settlement payout date?
The settlement administrator will distribute payments to approved claimants within 90 days after the court grants final approval of the settlement or 60 days after the approval of the claim, whichever is later.
Why is there a class action settlement?
The class action lawsuit alleged Boone Health’s use of third-party website technologies on its public website resulted in the transmission of certain information to third parties, which plaintiffs claimed was improper. Boone Health denied all allegations but agreed to settle to avoid the expense and uncertainty of ongoing litigation.
