Boone Health Patient Portal Privacy Class Action Settlement

Current and former patients who logged into the Boone Health patient portal between April 1, 2021, and Dec. 5, 2002, may qualify to submit a claim for a $20 cash payment and privacy monitoring from a class action settlement.

Boone Health Inc., d/b/a Boone Health, and CH Allied Services, d/b/a Boone Health and Boone Hospital Center, agreed to settle a class action lawsuit alleging it transmitted patient information to third parties via website technologies on its public website.

Who can file a claim?

Class members are current or former patients of Boone Health who logged into the patient portal at least once between April 1, 2021, and Dec. 5, 2022, and resided in Missouri during the qualifying period.

How much are settlement payments?

• Cash payment: Eligible class members who submit a valid claim can receive a cash payment of $20.
• Privacy Shield Pro: All class members will automatically receive a code to enroll for one year of Privacy Shield Pro, a digital privacy monitoring and identity protection service. Class members. donot need to submit a claim form to receive this benefit.

How to claim a settlement payment

To claim a settlement payment, class members can submit the online claim form or print and complete the PDF claim form and mail it to the settlement administrator.

The claim deadline is Nov. 9, 2026.

Settlement fund breakdown

The settlement fund includes:

• Settlement administration costs: To be determined

• Service awards to class representatives: Up to $2,000 each ($4,000 total)
• Privacy monitoring costs: Cost determined by the number of enrollments
• Payments to approved claimants: Total determined by number of valid claims filed

When is the Boone Health settlement payout date?

The settlement administrator will distribute payments to approved claimants 60 days after it approves the claim or within 90 days after the court grants final approval of the settlement, whichever is later.

Why is there a class action settlement?

The class action lawsuit alleged Boone Health used third-party website technologies on its public website that transmitted certain information to third parties.

Boone Health denied all allegations but agreed to settle to avoid the expense and uncertainty of ongoing litigation.
