Community Health Northwest Florida (CHNF), previously known as Escambia Community Clinics, is a healthcare provider serving the Pensacola Bay Area, including Escambia and Santa Rosa counties.

Founded in 1992, the organization operates a network of clinics and mobile units, offering services such as pediatric care, adult and family care, women’s care, dental care, behavioral health, pharmacy and optometry. The main site is located at 2315 W. Jackson St., Pensacola, FL 32505.

What Happened?

On Dec. 24, 2024, CHNF discovered suspicious activity within its network. The organization secured its systems and brought in third-party cybersecurity specialists to investigate. It was determined that an unauthorized party had gained access to the network and accessed files stored on certain systems.

The incident has been linked to a ransomware attack by a group called RansomHub, which claimed to have accessed 68 GB of the organization’s data and threatened to publish it on the dark web.

A comprehensive review was performed to identify the types of information exposed and the individuals affected. The investigation revealed that a wide range of personal and health information was exposed.

Information Exposed:

  • Names
  • Dates of birth
  • Social Security numbers
  • Driver’s license or state identification card numbers
  • Financial account numbers
  • Credit or debit card numbers
  • Patient identification and medical record numbers
  • Medical information
  • Health insurance information

The organization finalized the list of impacted individuals on Jan. 19, 2026, and began mailing notice letters to those affected. The total amount of individuals that were affected has not yet been released; however, the Vermont Attorney General was notified.

Your Rights and Next Steps

CHNF is offering at least 12 months of complimentary credit monitoring and identity theft restoration services through Cyberscout, a TransUnion company. You can enroll in these services by following the instructions in your notification letter.

It is recommended to remain vigilant by:

  • Regularly reviewing account statements and explanation of benefits forms
  • Monitoring your free credit reports for suspicious activity
  • Considering placing a fraud alert or credit freeze on your credit file with the major credit bureaus

If you notice any unauthorized activity or suspect identity theft, report it immediately to your financial institutions, the Federal Trade Commission.

You also have the right to file a police report if you become a victim of identity theft.
